Supplement your High Deductible Major Medical Policy with an Accidental Injury Plan

This Accident Only Medical Expense benefit covers up to $5,000 for all covered necessary medical and dental expenses per occurrence that are a direct result of an accidental injury. With a $100 deductible, this plan covers active members and spouses under age 70 and dependent children up to age 19. Coverage is extended to dependent children up to age 26 if they are a full-time college student. The package also includes a $10,000 AD&D benefit as outlined below.

$5,000 Accident Medical/Dental Expense Benefit and Accidental Death & Dismemberment
Accident Medical Expense Benefit: Pays the expenses you are charged by a Hospital, Doctor or certain other charges up to a maximum of $5,000, after a $100 deductible, if you are injured in a covered accident.
Accidental Death & Dismemberment: Pays the beneficiary up to $10,000 for the member’s death in a covered accident or a portion of that amount for loss of a certain part of the body.

These benefits are provided by Guaranty Trust Life Insurance Company, 1275 Milwaukee Avenue, Glenview, IL 60025 and are subject to the terms, definitions, conditions, exclusions and limitations, of the group policy. To obtain more information about this insurance, please contact the Association. L.I.F.E. Program members are eligible to receive these benefits by virtue of their enrollment in the L.I.F.E. Association.

Hospital Negotiation Services – Hospital Negotiation Services are performed by trained Financial Case Managers who work on a Member’s behalf to negotiate a reduced fee for hospital services. All services are negotiated on a case-by-case basis, and negotiations can be initiated with any hospital in the country. During 2007, when negotiations were initiated prior to services being rendered, the final cost to members was over 70% below the original price quoted by the hospital, while cases negotiated after the fact resulted in savings of nearly 50%. In order to secure such significantly reduced rates, Members must be capable of guaranteeing payment for all of, or a large portion of the negotiated fee at the conclusion of the negotiations. Hospital Negotiation Services are not available in Maryland and West Virginia or where prohibited by law.

Medical & Dental Financing – When a Member wishes to have an elective procedure performed, but is unable to raise the full amount due for the procedure, there is an affordable and easy financing option. Whether the medical procedure is cosmetic surgery, gastric bypass, vision correction, or any other elective procedure, a payment solution may be available.
